Operator ID Request Form

Please complete & submit the following form to request an Operator ID.

Please read the following information before completing the Operator Id Request Form. Submitting this on-line form is an acknowledgement that the information has been read and an agreement to follow the guidelines listed below.

Vehicles reserved through any of the Division of Fleet Operations Motor Pool locations shall be picked-up at the prearranged date, time and motor pool location. If the vehicle is not needed, the reserving person shall call to cancel the reservation prior to the prearranged date and time. If a reservation is not used and has not been canceled prior to the prearranged pick-up date and time the reservation shall be billed a $12.00 NO-SHOW charge.

Vehicles shall be returned to the correct motor pool location on or before the prearranged date and time. If the vehicle is required for a longer period of time, the reserving person shall call to have the reservation extended. If a vehicle is not returned or extended before the prearranged date and time, a daily LATE charge shall be added to the final billing.

When picking-up a daily use motor pool vehicle, the driver will be expected to show their driver's license to the Motor Pool Coordinator. Vehicles will not be released to any driver without a valid driver's license in their possession. In addition the driver shall read and sign an MP-98 form regarding the condition of the vehicle at time of pick-up.

When fueling a daily use motor pool vehicle, the driver shall enter the correct mileage. If the incorrect mileage is entered and can be traced back to the driver a $50.00 METER REJECT fee will be billed to the driver's account.

Vehicle maintenance and repair are coordinated through the ARI program. In the event a daily motor pool vehicle requires maintenance while out on a reservation, the driver should contact ARI for information regarding where to take the vehicle for service. If ARI is not contacted, the cost for repairs shall be billed to the driver's account.

If a vehicle is returned with one or more of the following items not completed, a $12.00 service fee shall added to the final bill:

§ Vehicles shall be returned to the correct Motor Pool location with a full tank of gas. If the vehicle is returned with less than a full tank of gas there will be a charge of $2.00 per gallon added to the final bill in addition to the $12.00 service fee.

§ Vehicles shall be parked in the area of the parking lot designated for motor pool use. Parking stall information shall be reported to the Motor Pool Coordinator. If there is a prearranged stall number assigned to the vehicle, it shall be parked in that stall upon return.

§ The correct ending mileage shall be reported to the Motor Pool Coordinator when returning the vehicle.

§ Each daily use motor pool vehicle is assigned a GasCard. If the GasCard is lost a $2.00 replacement charge in addition to the $12.00 service charge will be added to the final bill.

§ Vehicles shall be returned clean and in good repair. Vehicles not returned clean and in good repair shall be charged the detailing or repair costs in addition to the $12.00 service charge.
